This file will be stored in a secure, encrypted, password-protected electronic documentation system called OWL Practice, and will only be accessible by your therapist or another qualified therapist in the case of emergency or illness of your normal service provider. This platform complies with the Personal Health Information Protection Act (PHIPA), an Ontario law that governs the collection, use, and disclosure of personal health information to protect individuals' privacy. Additionally, our email communications utilize a confidential client number as a reference, ensuring the utmost privacy for individuals in our care. Nevertheless, it's important to note that no transmission method over the internet or electronic storage can be guaranteed entirely secure.

Any physical file content is stored in a locked filing cabinet in a locked room when not in use. We will destroy paper files and documents containing personal information by shredding, deleting it and when the hardware is discarded, ensuring it has been destroyed.

Your file will be stored for 10 years in compliance with health record maintenance requirements, or for 10 years after your 18th birthday, whichever is applicable

Your Rights:

You have the right to view your own records at any time and discuss any factual corrections after providing written consent. No information will be shared or transferred to another person without your explicit written consent.
